Mod/Admin applications + form

Well, I think there should be a mod/admin applications section. That's my first suggestion.

My second suggestion is the application form:

Recommendations from other players/mods/admins:
Have you ever been banned then unbanned(yes/no):
Have you broken any rules(be honest):
How have you contributed to the server/forums:
Are you a mod looking to be admin(yes/no):
How would making you mod/admin contribute to the server/forums:
Will you respect all players, staff or otherwise, explain:

By filling out this application I, [your in-game name here] hereby agree to abide by all forum/server rules, and agree to not abuse my powers in any which, way, or form. I also agree to respect all players, mods, admins, and especially owners. I solemnly agree to all terms above, or risk being demoted/banned.

If any player disagrees with [your in-game name here] becoming a member of staff, please state so with a complete explanation of why you oppose the idea of this player becoming mod/admin. If you are caught lying because you dislike this player, or for any other reason, action will be taken against your account if necessary.

I have my own made application, plus there not open right now.
